Use this form to submit announcements/event information. All announcements must be submitted no less than 7 days before the day you would like it announced or posted. For publication in the monthly Newsletter, you must submit your information by the 23rd of the month prior to the event.

DIRECTORY INFORMATION & UPDATE FORM

If your information for the church directory changes, click on the Form below and submit to the church office. Electronic submission is encouraged

Information Update Form

SPACE USAGE REQUEST FORM

Use this electronic submission form to request permission to reserve church facilities for different events. Requests must be received at least 24 hours in advance of requested usage, or if it's a weekend usage, the request must be in the office by 5pm Thursday. MUST RECEIVE OFFICE APPROVAL PRIOR TO USAGE.

Please Note - This form is only for UCJC-related ministries, events, and meetings.
